Online Registration: Please contact your child's YCSD school building if you need help completing the online registration on Final Forms.

Enrollment/Registration

All students should be registered by their parent(s) or court-appointed legal guardian. A court-appointed guardian should present guardianship papers at the time of enrollment. In the case of divorce, the custodial parent should provide documentation showing custody.

Within 24 hours of a student's entry into school, a request for records from their previous school must be made. (ORC 3313.672 (A)(3)).

Enrollment/Registration for the 2024/2025 school year is open!

Registration for all grade levels must be completed through the district's online enrollment system Final Forms using the link below. Returning families must log in to Final Forms and resign all forms for the new school year.

Students new to the district must upload the following documents to Final Forms upon registration:

  • An official copy of the student’s Birth Certificate,

  • Complete Immunization Records

  • If transferring from another Youngstown City School, a Transfer Slip from the sending school is required.

  • Proof of Residence:

    • Home Owners: A property deed or signed purchase agreement.

    • Renters: A signed formal lease agreement (rent receipt is not acceptable).

    • Non-Renters: (Both of the following are required)

      • A notarized letter, signed by the property owner, verifying the occupancy of the parent or guardian and the student.

      • Mailing Address Verification. Examples include unpaid or current utility bill(s), voter registration, ADC medical card (showing address) for the current month. Welfare recipients must have proof that their account has been changed to their new address.

  • Caseworkers or Foster Parent

    • The Custodial Information for the caseworker/agency should be entered.

    • The Legal Parents/Guardians should be entered into the Contact Information

    • If there is a court decision or a placement decision, or certificate of custody – each

      document should be uploaded as Other Documents

    • Foster Parent information is considered Family/Guardian #1 information

    • The Foster Parent should provide the proof of residence listed above.

    • The email address of the foster parent is considered the parent email address.

    • A required Form Signature is needed to move on.

  • Open Enrollment Application

    • This is used ONLY for students who are enrolling into Youngstown from a local area

      school district such as Liberty, Austintown, etc.

    • Note – open enrollment into Youngstown City must be approved by Culture and Climate

      personnel and the district Superintendent.

    • Once approved, the HOME district is notified of the student placement by the EMIS

      department.
